A shell is whatever you want it to be. There are no hard and fast rules. I think 4 to 8 oz is average. I tend to go for about 8 oz so there is a bit more liquid rather than a sludgier 4 oz.

It's a personal choice depending on whether you want to dilute the taste and drink more liquid, keeping in mind that kava is a diuretic. My preference is 140ml per shell. Easy to down that amount in one take. The 'shell' refers to a half coconut shell that is traditionally used as the drinking vessel.

Less than 16oz. 500ml is a lot of liquid to introduce at once. If you did this with water, it may upset your electrolyte balance. I enjoy 300 to 400m in a few gulps, a savory, listening gulp, crafted with about 6 grams kava root per 100ml water and a ten minute brewing time with the cloth strainer.
